ACE Application Check List
- Ensure all planners/speakers/moderators etc. disclose all relevant financial relationships
- Attach all CVs for everyone associated with the activity/conference
- Attach all supporting documentation for the activity/conference
- i.e. articles or hospital data reports to support the needs assessment for this activity/conference
- Learning objectives (Max 20)
- Brochure with the mandatory requirements (for conferences/symposiums/live activities):
- Title and date of the activity
- Registration and payment information
- Speakers with credentials and place of employment i.e. UAB, Division of Endocrinology
- Full Agenda with beginning and ending times
- Learning Objectives
- Designation Statement
- Accreditation Statement (this will be different for a joint activity i.e. with the AL Chapter AAP)
- Maintenance of Certification (MOC) information if applying for this type of credit
- List of anticipated commercial supporters if applicable
- List of Exhibitors and a copies of the Exhibitor Contracts
- Preliminary Budget if applicable
