Care Coordinators

Care Coordinators in the Cardiovascular service line are here to support patients and families throughout their hospital stay and beyond. Understanding that hospitalizations can be both planned and unexpected, our care coordinators work closely with patients during their stay to ensure a smooth transition between levels of care. They facilitate comprehensive discharge planning, including the coordination of home medical equipment and medications, outpatient follow-up appointments, and personalized, hands-on education tailored to each patient’s unique needs. Their ultimate goal is to equip families with the knowledge and confidence necessary to provide safe, effective care for their loved one after discharge.
