Adolescent and Young Adult Diabetes Clinic

BRIDGE Clinic: Building Readiness, Independence, Diabetes Guidance & Empowerment

Our clinic provides a multidisciplinary approach focused on helping adolescents and young adults build the skills and confidence needed to manage their diabetes and overall health. During visits, patients are encouraged to take an active role in their care, including spending part of the appointment independently with their healthcare provider. Patients will also work closely with a collaborative team that may include physicians, dietitians, diabetes educators, and social workers. Together, the team supports the development of self-management skills, healthcare navigation, and readiness for adult-centered care.

  • Adolescent and early young adults with Type 1 and Type 2 diabetes

Clinic Location and Schedule:

Meets once every other month on Friday afternoons.

Scheduling an Appointment

To schedule an appointment, please call our clinic at 205-638-9107.
Please schedule 2–4 hours for initial evaluation. Evaluation includes history gathering, full physical examination, blood or urine testing
