Registration and Directions


Patient Information
If your child is a new patient to Pediatric ENT Associates of Alabama, there are several pieces of paperwork you will be ask to turn in the day of your visit. You can fill these out in the office, or you can print them in advance and bring them already filled out to you visit.

Online Pre-Registration
At Pediatric ENT Associates we recognize that your time is very valuable and sitting in a lengthy doctors visit is not a pleasent experience. We want to make your visit with us as effecient as possible. One of the ways we are trying to speed up the process is allowing you to be able to Pre-Register online at least 24 hours prior to your child's appointment. The clinic registration process is a lengthy process if done entirely in clinic. By going online and completing the pre-registration process you will be able to expidite your visit.

Clinic Locations and Directions
Pediatric ENT cares for patients at two Children's of Alabama campuses in Birmingham. Please clarify which location your child's appointment or surgery is scheduled for.

Patients that are scheduled at Children's South will see their surgeon in Clinic F on the second floor. If surgery is scheduled for Children's South, One Day Surgery is also located on the second floor.

Patients that scheduled for surgery at the Benjamin Russell Hospital for Children should follow the directions below for parking, and to find the One Day Surgery center which is located on the third floor.

Patients who have a clinic appointment with one of the Pediatric ENT Assoicates doctors at the downtown location will be seen in the McWane building of Children's of Alabama, in clinic 4.
