New Patient Form

Please call one of our offices first to discuss becoming a new patient.  Once you have been confirmed as a new patient at your chosen location, we will work with you to set up your child's first office visit.

We will have a lot of questions for you.  To make that first visit as smooth as possible, we suggest that you print our General Information form, complete it at home and bring it with you to your child's first appointment. You may also need to complete the Consent to Treat form. Doing this will save you some time in our reception area and will get you and your child home sooner to play!
