CHADIS Questionnaires

CHADIS is an online tool used to provide families a way to fill out questionnaires before the visit. We choose specific questionnaires to help us get to know your child and assist with the early diagnosis of developmental, behavioral, and health issues.

CHADIS will save you time during the office visit. Completing the questionnaires electronically before your visit allows the doctor to better prepare prior to seeing your child in clinic, and this can help decrease your wait time in our office.

You will receive a CHADIS text notification on the Monday before your scheduled visit. Your doctor will have access to your confidential results after you complete and submit each form. These results will be discussed during your visit.

New Patients

1. Fill in your information and create a Password to complete your registration. (If you do not have an email address, CHADIS will create a username for you)
2. Enter your child’s information (Remember this information is kept private and is HIPAA protected. Your information will be stored securely)
3. Select Take Questionnaires
4. Complete the questionnaires.
5. Don’t forget to click on Send It; I’m Done at the end of each questionnaire.

Returning Patients

1. Select Take Questionnaires
2. Complete the questionnaires.
3. Don’t forget to click on Send It; I’m Done at the end of each questionnaire.
