Attestation information

All physicians wishing to receive credit must attest to meaningful participation and complete a Physician Attestation form and submit to the project leader for review and approval. All attestations are also reviewed by the Children's of Alabama Portfolio Program. Meaningful participation is defined as involving both an active role in the project, and participation over an appropriate period of time in implementing a change. At a minimum, the activities must be of sufficient duration to allow for participation in at least one full cycles of data assessment and planning of an improvement intervention, the implementation of a change process and re-assessment of the results (e.g., PDSA cycle) for at least six months duration.

At least 3 of the 4 must also be met:

  • Involved in initial project design, including but not limited to identifying the gap in quality, development of primary outcome measures and target improvement, and development of plans for intervention
  • Involved in implementation of strategies and interventions
  • Involved in data collection and/or analysis to assess the impact of the interventions while making appropriate course corrections in the improvement effort
  • Participation in at least three team meetings to continue the process for improvement (documentation of meeting attendance and minutes must be available upon request). At least one meeting should involve evaluating a chart of data to assess the impact of the intervention

The participant is able to personally reflect on the activity, describing the change that was performed in their practice and how it affected the way care is delivered.

Once your project is complete, we will submit your final project to the American Board of Pediatrics. This review may take to 2 weeks. After it is approved, the project team members listed for the project will be notified and instructions for downloading and completing the attestation form will be emailed.
