Wills and Bequests: Include Children’s Hospital of Alabama in your will:
Sample language: I hereby give to Children’s Hospital of Alabama, Tax ID No. 63-0307306 the sum of $_____, for its general purposes.
Giving through life insurance: Name Children’s Hospital of Alabama as the beneficiary, or donate the policy.
If you have a life insurance policy that you no longer need because of a change in your life circumstances, you can donate the policy to Children’s Hospital of Alabama, even if you are still paying premiums. In the alternative, you can simply contact your life insurance company and designate Children’s as the beneficiary of the benefit.
Giving through retirement plans: Name Children’s Hospital of Alabama as the beneficiary.
The beneficiary designation is a contract between you and the plan administrator. You should contact the administrator for a ‘change of beneficiary’ form.
Giving through bank accounts/mutual funds:
